Business Recovery Grant

Please submit the following documents to be considered:

1. Completed Grant application

(blank copy available for download below)

 

2. Copy of Business License

 

3. 2019 tax return (Schedule C page) or the corresponding year business opened (business must have opened for business no later than June 30, 2021)

 

4. Completed W-9

(included with application packet & available for download below)

 

5. Covid-19 Hardship Letter
